You should only be filling this form out if you purchased additional ice from a facility and setup payment as DIRECT BILL to RAHA.  If you paid for your ice directly to the arena by check or credit card DO NOT fill out this form.  You should add the cost into your team budget and write a check back to yourself for reimbursement.
